89 years old client needs assistance with toileting, has slight dementia, gets a little more confused at night. She lives in a separate in-law apartment from her son, has own kitchen, bedroom, and bathroom. Son prefills medications in pill box, caregiver to remind to take them. No pets, no smokers.
Client is an 84 year old male who lives in his own home. Client has difficulty breathing, is on oxygen.He walks with a walker, needs encouragement to get up and walk short distances throughout the day. Client's home is very handicap accessible. He is incontinent, caregiver is to encourage a toileting schedule to decrease incontinence. Daughter visits often, brings 2 small dogs to visit sometimes. Client also has a wheelchair to use. Client goes to bed at around 8:00p and wakes at 9:00a/10:00a. Client likes to be showered every day, lotion his body, NOT his feet, assist with toileting, client has cirrohosis, (skin condition). Client has nebulizer treatments in the morning and night, also has 2 puffs of SPIRIVA mid day. Daughter or caregiver will do grocery shopping. Caregiver to make all meals, and eat with client, client enjoys all foods, just not spicy. Fish, chicken, red meats. There will be hourly caregivers that client had in the past that will sit with him for 3 hours 1-2 times a week, caregiver can go out, or do personal things at this time. Client's bed is changed everyday.
Schedule is Friday 9 am until Monday 9 am (3 full live-in days)
