Client is 90 years old, walks with a walker, has poor balance, forgetful, very pleasant, wonderful, kind, fun loving woman. Caregiver to provide personal care, meals, initiate exercise, walk around their house property which is on the water. Client is not incontinent, but wears a pad as a preventative. Client is agreeable to care. Needs assistance getting from sitting to standing position. Client goes to church on Sundays, her friend takes her, but caregiver needs to come to assist getting in and out of the car.
Client has dysphasia (inability to get words out), she sleeps at night, she is very healthy. Client walks independently, needs personal care assistance, wears depends, meal preparation, medication reminders. There is a neighbor who will stay with a client while caregiver does grocery shopping. Client gets tired easily, caregiver to encourage/cue client to finish her food, drink, etc. Caregiver will have their own bedroom, must have Drivers License and clean driving record, client has a car, there is a visiting dog (a collie).
A very sweet woman, who has Multiple Sclerosis and recently had gone through the surgery on her spine, needs help 3 times a week in Durham, CT. She does NOT walk and she uses a sliding board to transfer. Occupational Therapy is working with her to help her get better at transfers. She lives with a husband who goes to work full time. There is a friendly poodle dog. Caregiver to make breakfast, laundry, and help with personal care and transfers. Client's weight is 120 lb and she is 5'5" tall.
